REGISTRATION

To register your child at Piñon Unified School District #4 using the Online Enrollment Portal, you must have a email address and register for a Online Enrollment account.

Use the Registration link and complete the registration online. You will need to be prepared to provide emergency contacts and medical information.

You will also need to bring original documents to your child's school. We’ll make copies and return the originals back to you.

IMMUNIZATION

Arizona state law requires children attending school to provide proof of immunization at the time of registration. Subject to state policy A.R.S. 15-873, we may deny attendance of a student not meeting state immunization or exemption requirements.

Returning Students Required Documents:

  • Arizona Residency Form - please submit a new residency form if your residency has changed from SY23-24.

  • Updated Immunization - an updated immunization may be required when child requires new vaccinations.

A SNAPCODE is required to see your child's enrollment forms.
The snapcode will be provided by your child's school.

  • Report Card/Transcript with grade placement for SY 2024-2025

  • Withdrawal Form

  • Certificate of Indian Blood

  • Birth Certificate

  • Updated Immunization Record

  • Current Physical Address Document

  • Guardianship Documents (if applicable)

How do I create a Parent PowerSchool account?

The Online Enrollment Portal does not create a Parent PowerSchool account for you. To create a Parent PowerSchool account, use this link here.

Contact your child's school for the access ID and password.

The Parent PowerSchool portal will allow you to stay up to date with your child's grades, attendance, and assignments. And in the future, you will be able to submit online enrollment forms.
